Minor piercing consent
This summarizes the minor consent and release. It does not submit anything. The parent or legal guardian completes the live form on the original studio site.
The form still says Piercings Only. The public name is Stickem Up Piercings. Minors 12 and older need a parent or legal guardian and documentation. Dermals and below-the-belt piercings are 18 and older, with no exceptions.
What the form says
- By submitting, you agree to the terms and state that you are the parent or legal guardian.
- You consent to the named piercing artist performing the piercing on your minor child.
- You release Piercings Only and the piercing artist from liabilities, claims, actions, and demands that your child might have because the studio did the piercing you requested.
- Employees and independent contractors are not acting as medical professionals. Suggestions are not medical advice.
- The piercing uses professional techniques and professional instruments. Infection can follow poor aftercare, poor hygiene, or jewelry sensitivity.
- You agree that your child will follow the written and spoken aftercare until healing is complete. Healing may take several months.
- There is no refund for any reason.
