Piercing consent
This is a summary of the adult consent and release so you can read it first. It does not submit anything. The live form is on the original studio site.
The form still uses the older name Piercings Only. The public name of the studio is Stickem Up Piercings.
What the form says
- Submitting the form means you agree to the terms and consent to the named piercing artist performing the piercing.
- You release Piercings Only and the piercing artist from liabilities, claims, actions, and demands connected with the piercing. The adult form's release sentence uses the words "my child," the same wording as the minor form.
- Employees and independent contractors of Piercings Only are not acting as medical professionals. Their suggestions are not medical advice and are not a substitute for a clinician.
- The piercing is performed with professional techniques and professional instruments.
- Infection can happen if aftercare or hygiene is not followed, or because of jewelry sensitivity.
- You agree that written and spoken aftercare instructions will be followed until healing is complete. Healing may take several months.
- There is no refund for any reason.
